Authentication failed at VPN and Userportal with MFA enabled

Check the NTP Server settings of the Firewall and the actual time.

The time could have changed (new ntp server or resync) and so the MFA-Key is asynchron, although the token time offset tells you, everything is ok.
Delete the MFA-Code and tell the user to get a new one over VPN/Userportal.
